gun research [out of control]

Nature in its 06 October 2022 edition has an editorial calling for a public-health approach to gun terrifying mortality in the US. Which makes it such an outlier among high income countries. Not the first time in this journal, as legal ban on research on the topic has been systemic for many years.

“Given the prevalence of firearm injury and death in the United States, it is astounding that scientific understanding of the problem is so poor and that the research infrastructure and workforce are so underdeveloped.”

The tribune lists how interventions could reduce gun fatalities and address violence. However, it never mentions the elephant in the room, namely the legality of buying and carrying weapons on a scale with no comparison with other developed countries. This may be what they call “shift[ing] our national view of firearm-injury prevention away from a political issue”, but advocating treating the effect rather than the cause is delusional. (To be fair, Nature had another news article this year calling for scientific research into gun control.)

While the special health council for the French Government is closing down, the number of cases here remains quite high, seemingly in a complete indifference or worse… When 25 000 persons died from COVID since January. But political parties are all (!) against any constraining measure, with some even calling in a most demagogic manner for reintegrating [the few hundred] unvaccinated public health personnel in the public health system… (As a single datapoint, take the counter-example of our thrice-vaccinated daughter who caught COVID last week, most likely when working at the hospital.)

Nous, acteurs de la lutte contre le Covid, nous voterons sans hésitation contre Marine Le Pen, en mettant dans l’urne un bulletin Emmanuel Macron le 24 avril. Notre vote sera d’abord un combat contre l’exclusion, le sectarisme, l’intolérance, le racisme et la xénophobie, qui ne peut souffrir aucune abstention. Les combats que nous avons menés et que nous mènerons encore pour que les principes d’expertise scientifique, de justification publique et de précaution sanitaire soient mieux respectés par l’exécutif ne nous détourneront pas du seul objectif qui nous semblera absolument prééminent le jour du vote : faire barrage à l’extrême droite.

Nous sommes du côté de ceux dont le nom est difficile à prononcer et nous savons que sans les soignants étrangers, le système de santé français ne tiendra pas. Nous savons ce que nous leur devons.

Oui, notre combat contre le Covid nous a souvent conduits à dénoncer certains choix gouvernementaux que nous jugeons imprudents. Au nom des cent personnes qui, en France, meurent actuellement chaque jour du Covid, au nom des 1 700 patients qui entrent à l’hôpital quotidiennement, au nom des 20 000 enfants qui ont combattu le Covid à l’hôpital depuis mars 2020 après y avoir été souvent exposés dans leurs classes, au nom aussi de tous nos compatriotes affectés par des symptômes persistants du Covid dont l’exécutif paraît encore tout ignorer : nous pensons que l’optimisme qu’affiche désormais Emmanuel Macron face à ce virus est déplacé. Au nom de nos expertises respectives, en tant que soignants et en tant que chercheurs, nous dénonçons la trop faible détermination dont l’exécutif a fait preuve pour combattre le charlatanisme, prêtant une oreille souvent bien complaisante à ceux qui discréditent la connaissance scientifique et prêtent au masque ou au vaccin des vices imaginaires. Pour nous, la fin de l’obligation du port du masque en lieu clos aurait dû s’accompagner d’une pédagogie robuste et déterminée incitant chacun à continuer à le porter par souci de soi et des autres, alors que les messages individualistes laissant aux plus fragiles le soin de se protéger eux-mêmes nous révoltent. Aussi, nous attendons encore un investissement conséquent dans la qualité de l’air intérieur. Nos valeurs de solidarité avec les plus fragiles, notre confiance dans la science, le soin et la raison, notre foi dans l’action collective au service du bien commun guident notre engagement contre l’épidémie aussi bien que contre la stratégie actuelle de circulation virale sans freinage, adoptée sans justification publique et sans débat démocratique.

Sur ces combats, nous ne lâcherons certes pas. Mais pour ces combats, aussi, nous n’aurons aucune hésitation à faire barrage à la candidate d’extrême droite en mettant dans l’urne un bulletin pour Emmanuel Macron le 24 avril. «Objectivement, je pense que la crise aurait pu être très largement mieux gérée par Donald Trump», a-t-elle déclaré sur France Inter le 10 juin 2020. Comme avec son ami Bolsonaro, Marine Le Pen, face à la pandémie, ce serait la porte ouverte aux charlatans, l’incompétence et l’inconséquence, l’invocation sophiste de la liberté individuelle au mépris des plus vulnérables, le discrédit permanent jeté sur l’expertise et le progrès scientifique au profit de fascinations complotistes assumées. Ses valeurs la portent davantage à placer, comme elle a tenu à le faire savoir, un santon de Didier Raoult dans sa crèche de Noël et à acheter des vaccins russes qui n’ont jamais démontré leur efficacité, qu’à protéger nos enfants, nos âgés, nos concitoyens immunodéprimés face aux prochaines vagues.

Et ce ne sont, bien sûr, pas seulement les principes de la santé publique que l’extrême droite piétine. Pour nous, à l’heure du vote, il n’y aura pas de quitus à l’égard de l’exécutif, mais rien ne saurait pour nous justifier l’abstention face à un péril de cette nature.

‘Statistics have been crucial both to our understanding of the pandemic and to our efforts to fight it. While we hope we won’t see another pandemic on this scale, we need to see a culture change now – with more transparency around data and evidence, stronger mechanisms to challenge the misuse of statistics, and leaders with statistical skills.’

  • Invest in public health data – which should be regarded as critical national infrastructure and a full review of health data should be conducted
  • Publish evidence – all evidence considered by governments and their advisers must be published in a timely and accessible manner
  • Be clear and open about data – government should invest in a central portal, from which the different sources of official data, analysis protocols and up-to-date results can be found
  • Challenge the misuse of statistics – the Office for Statistics Regulation should have its funding augmented so it can better hold the government to account
  • The media needs to step up its responsibilities – government should support media institutions that invest in specialist scientific and medical reporting
  • Build decision makers’ statistical skills – politicians and senior officials should seek out statistical training
  • Build an effective infectious disease surveillance system to monitor the spread of disease – the government should ensure that a real-time surveillance system is ready for future pandemics
  • Increase scrutiny and openness for new diagnostic tests – similar steps to those adopted for vaccine and pharmaceutical evaluation should be followed for diagnostic tests
  • Health data is incomplete without social care data – improving social care data should be a central part of any review of UK health data
  • Evaluation should be put at the heart of policy – efficient evaluations or experiments should be incorporated into any intervention from the start.
